As a member of the Pennsylvania Association of Community Health Centers (PACHC), Community Health Net offers a range of quality healthcare services for newborn through geriatric patients. Including medical, dental, vision, gynecology, Ryan White HIV-Aids Medical Services, Health Care for the Homeless and a full service 340B pharmacy, Community Health Net accepts all patients, regardless of their ability to pay.

Originally incorporated in 1985 as Primary Health Services of Northwestern PA, with a small medical and dental office inside of the Booker T. Washington Center, Community Health Net has transformed into a community leader, serving as a single coordinator for medical clinics within the community. On July 1, 1985, Primary Health Care Services was certified as a Federally Qualified Health Center (FQHC) under the Public Health Services Act. It wasn't until ten years later when collaborations between other community and healthcare facilities began.

Collaborations with Saint Vincent Health Center, Hamot Medical Center, Metro Health, Booker T. Washington Center, Martin Luther King Center, John F. Kennedy Center and the Hispanic American Council began in 1994, in order to establish a uniformed restructuring of the area's healthcare services provided to the uninsured and underserved population in Erie County. With this strategic approach, the collaborative goal was achieved in 1995, and Community Health Net was born. In addition to the Booker T. Washington Center, Martin Luther King Center, and John F. Kennedy Center, new satellite offices were opened at the Hispanic American Council, Metro Health Center, Saint Vincent Health Center and Hamot Medical Center.

In July 2004, Community Health Net consolidated their medical practice to the Daniel S. Snow, MD Health Center, 1202 State Street and the dental practice to the Booker T. Washington Center located at 1720 Holland Street. “The organization received recognition from the U.S. Public Health Service and the U.S. Department of Health and Human Services for?its innovative approach in developing an integrated service network and expansion among community partners,” adds Dr. Snow.

In 2010, in collaboration with the Lake Erie College of Osteopathic Medicine (LECOM) School of Pharmacy, CHN opened its 340B Pharmacy.

This community-wide effort with our partners provides access to healthcare for Erie's medically underserved and uninsured populations. “Established on a philosophy of respect and dignity, our overall goal is to deliver a safe and timely process of care and treatment,” says Dr. Snow. This ideal healthcare process attains progressive effects for patients' health. Highly dedicated staff at Community Health Net provide extraordinary quality healthcare to all of their patients, while successfully navigating the challenges and opportunities of healthcare reform.

“Whether you require medical assistance, preventative healthcare, health education, outreach or referrals, we are here for you,” says Dr. Snow. “We accept all insurances including Medicaid, Medicare and CHIP. We also offer our patients Sliding Fee Discounts based on the Federal Poverty Guidelines. We are willing to work with you to provide the best possible healthcare available.”
